Do you provide instructions completing USPS Form 1583?

Yes, however to make the process as easy as possible, we prepare the required USPS Form 1583 for you once you complete the sign up process. Please note that you must complete the online sign up to secure your mailbox number prior to completing USPS Form 1583.

  • Box # 1Enter todays date.
  • Box # 2Enter the name of the individual who will be receiving mail. If mail will be sent to a business name, enter the business name here as well. Can use a comma or slash ( / ) between the individual name and business name.
  • Box # 3
    • (a)Enter the PMB or mailbox number you selected during signup.
    • (b)This should be pre-filled with the appropriate City for the Freedom Mailbox® location you selected during signup.
    • (c)This should be pre-filled with the appropriate State for the Freedom Mailbox® location you selected during signup.
    • (d)This should be pre-filled with the appropriate Zip Code for the Freedom Mailbox® location you selected during signup.
  • Box # 4
    • (a)This should be pre-filled with the name Freedom Mailbox.
    • (b)This should be pre-filled with the appropriate Street Address for the Freedom Mailbox® location you selected during signup.
    • (c)This should be pre-filled with the appropriate City for the Freedom Mailbox® location you selected during signup.
    • (d)This should be pre-filled with the appropriate State for the Freedom Mailbox® location you selected during signup.
    • (e)This should be pre-filled with the appropriate Zip Code for the Freedom Mailbox® location you selected during signup.
  • Box # 5Sign in box 5 if you wish to give Freedom Mailbox® staff permission to receive restricted mail. Restricted Delivery is designed specifically for sensitive mail such as confidential legal documents, important contracts, classified documents, and high-value packages or papers.
  • Box # 6Enter the name of the applicant, usually the same name of the individual provided in Box #2.
  • Box # 7
    • (a)Enter the Home Street Address of the applicant provided in box #6. Can not be a PO Box.
    • (b)Enter the City of the home address provided in box #7a.
    • (c)Enter the State of the home address provided in box #7a.
    • (d)Enter the Zip Code of the home address provided in box #7a.
  • Box # 8Leave this blank. The notary usually fills Box 8a and Box 8b as they verify the forms of identification provided.
  • Box # 9Any business use of the mailbox requires the business name to be listed. Only fill in this box if applicable.
  • Box # 10 (only applies if box 9 was filled in)
    • (a)Enter the Street Address of the business provided in box #9.
    • (b)Enter the City of the business address provided in box #10a.
    • (c)Enter the State of the business address provided in box #10a.
    • (d)Enter the Zip Code of the business address provided in box #10a.
    • (e)Enter the Telephone Number of the business provided in box #9.
  • Box # 11Enter the type of business provided (e.g. Consulting Services). Only applies if box 9 was filled in.
  • Box # 12If you wish to receive mail for other people at your company (e.g. members or employees), add their names here. Although each name listed must have verifiable identification, a separate Form 1583 for each person is not required unless requested.
  • Box # 13If your business is a Corporation, enter the names and addresses of all officers.
  • Box # 14If your business name is registered, enter the county, state and date of registration. You may need to reference your DBA or doing business as files, also called a fictitious business name in some states.
  • Box # 15Leave this box blank as it is for the notary to sign.
  • Box # 16Your signature is required.
